Behavior of anti-HLA antibodies in patients of the Cuban Renal Transplant Programs

Introduction: Cuban patients from the Living Donor Kidney Transplant Program (TRDV) and Cadaveric Donor Program (TRDC) were studied for the presence of anti-HLA antibodies. Objective: To compare the population allo-sensitization and cPRA values of TRDV and TRDC . Methods: Anti-HLA antibodies were detected and characterized in 291 TRDC candidates and 74 TRDV candidates using Luminex xMAP technology, from February 2017 to May 2019, in the Histocompatibility Laboratory of the Institute of Hematology and Immunology,. The frequency of donor-specific anti-HLA antibodies (DSA) in the TRDV and the dependence between the type of program, anti-HLA antibodies and the calculated panel reactive anti-HLA antibodies (cPRA) were analyzed. Results: Allo-sensitization was low in the TRDV and high in the TRDC. Class II antibodies predominated. The TRDC had a higher proportion of these antibodies. 41.6% of TRDV anti-HLA positive patients had DSA. In both programs, more than two thirds of the anti-HLA positive patients had a cPRA greater than 20%. Conclusions: TRDC have higher population allosensitization and cPRA values than TRDV, although the pre-transplant presence of elevated DSA and cPRA in the latter is not unusual.Downloads
