Characterization of anti-HLA antibodies in Cuban candidates for allogeneic hematopoietic progenitor cell transplantation

Abstract

Introduction: The detection and characterization of antibodies against the main histocompatibility system (anti-HLA antibodies) in the candidates to allogeneic hematopoietic progenitor cell transplantation (allo-HSCT) is one of the tests that ensures donor-recipient compatibility.

Objective: To characterize the pre-transplant alloimmunization with anti-HLA antibodies of the Cuban candidates for allo-HSCT.

Method: 111 samples from allo-HSCT candidates and 339 of their possible donors, received from April 2017 to March 2022 at the Histocompatibility Department of the Institute of Hematology and Immunology, underwent detection and characterization of anti-HLA antibodies and HLA typing, respectively, by Luminex xMAP technology. The existence of an association between the variables sex and age with the presence of anti-HLA antibodies, their classes, percentage of anti-HLA antibodies compared to the calculated panel (cPRA) and the presence of donor-specific anti-HLA antibodies (DSA) was analyzed.

Results: Population allo-sensitization with anti-HLA antibodies was 26.1%, of which 62.1% had DSA, mostly class II. Although there was a predominance of the male sex, women presented a higher frequency of these antibodies overall and by class, DSA and cPRA higher. There was no association of these antibodies with age.

Conclusions: The allo-sensitization of the Cuban population in candidates for allo-HSCT is low, but women are more sensitized, so they have less possibilities of finding compatible donors.
