Monitoring of acute renal dysfunction in severely ill pediatric patients diagnosed with acute lymphoid leukemia

Abstract

Introduction: Acute kidney dysfunction is a syndrome that occurs due to multiple causes. It is characterized by the abrupt decrease in glomerular filtration, which results from the inability of the kidney to excrete nitrogenous products and to maintain the homeostasis of fluids, electrolytes. It is frequent in pediatric patients diagnosed with acute lymphoid leukemia.

Objetive. To evaluate acute renal dysfunction in severely ill pediatric patients diagnosed with acute lymphoid leukemia according to the scale of risk, damage, failure or prolonged loss of renal function.

Methods. A multicenter, observational, prospective study was carried out in severely ill patients with acute renal dysfunction and acute lymphoid leukemia, in intensive care units of Cuban hospitals with a total of 92 patients.

Results. A greater representation of the damage was observed in children under one year of age and those between 5 and 9 years of age in males. The risk factors that were most related to stage I of the RIFLE risk scale for acute renal dysfunction upon admission with acute lymphoid leukemia were transfer to the pediatric intensive care unit, hypovolemia, metabolic acidosis, use of vancomycin and mechanical ventilation.

Conclusions. The study demonstrated the importance of adequate monitoring of acute renal dysfunction in patients with acute lymphoid leukemia; Although an association with the use of chemotherapy was not evident.
