Cytogenomic alterations in Cuban pediatric patients with B cell acute lymphoblastic leukemia

Abstract

Introduction: B-cell acute lymphoblastic leukemia is the neoplasm that most frequently affects children. Cytogenetic and molecular alterations are important prognostic biomarkers that support risk stratification at diagnosis and the most appropriate treatment scheme.

Objective: To characterize the cytogenetic and molecular biomarkers in Cuban pediatric patients with B-cell acute lymphoblastic leukemia.

Methods: An observational, retrospective, multicenter cohort study was conducted between January 2012 and December 2020. A sample of 84 patients was taken from the universe of Cuban pediatric patients diagnosed with B-ALL, who received treatment at four oncohematological centers in the country.

Results: The analysis of numerical alterations showed a predominance of patients with high hyperdiploidy. The molecular study showed that the ETV6-RUNX1 mutation was the most frequent in 28.6 %. Age, sex, and skin color were not associated with the biomarkers evaluated. The BCR-ABL gene was related to morphological response variables to induction therapy (p=0.04). Hypodiploidy and t(9; 22)/BCR-ABL were predictors of survival.

Conclusions: The evaluation of cytogenetic and molecular biomarkers in B-cell acute lymphoblastic leukemia is a fundamental pillar in the application of risk-adapted treatment protocols as part of the development of personalized medicine in the country.
