Common variable immunodeficiency: diverse phenotypic expression in a series of cases

Introduction: Common variable immunodeficiency is the largest group of symptomatic primary immunodeficiencies, with variability in its phenotypic expression. It is concomitant with allergies, autoimmunity or neoplasms. It is associated with infections and low levels of two or more immunoglobulin isotypes. Health professionals relate immunodeficiencies to infections, so it is important to divulge the variable phenotypic expression that exists in the immunosuppressed.
Objective: To describe the variations in the phenotypic expression of common variable immunodeficiency in patients treated in Granma between 2012 and 2022.
Method: Five cases were reviewed and contrasted in terms of clinical manifestations and results of complementary studies.
Results: Low IgG and IgA values were found. The age of debut occurred in late childhood and after 50 years. In one case infections predominated, in another pernicious anemia and multiple myeloma and cytopenias (thrombocytopenia and anemia), splenomegaly and celiac disease appeared as a manifestation in the series. The CD19+ population was above 1% in all cases and in one case the CD3-CD16+CD56+ population was decreased. Expanded phenotype studies by flow cytometry placed two cases according to the EUROClass classification in the smB+ CD21norm Group: LB >1%, >2% post-switched memory B cells and no LB-CD21low expansion.
Conclusions: The presence of infections, autoimmunity and tumor diseases in the series of cases studied demonstrates the phenotypic heterogeneity of common variable immunodeficiency.
