Citometría de flujo en pacientes con anemia hemolítica autoinmune caliente con prueba de Coombs negativa

Abstract

Introduction: Autoimmune hemolytic anemia is a disease characterized by the presence of antibodies directed against the patient's own red blood cell antigens. Immunohematological diagnosis is primarily based on evidence of autoantibodies in red blood cells using the direct antiglobulin test (DAT); however, this test is occasionally negative.

Objective: To apply flow cytometry to diagnose PAD-negative warm autoimmune hemolytic anemia at the Institute of Hematology and Immunology.

Methods: An exploratory, cross-sectional, research-development, and technology evaluation study was conducted. Red blood cell suspensions from 72 patients with suspected autoimmune hemolytic anemia were analyzed at the Institute of Hematology and Immunology between 2020 and 2024.

Results: Of the 72 samples studied, 35 (48.6%) were female and 37 (51.4%) were male. Autoantibodies were detected in 39 samples, 54.16% of the total studied. Of these, 28 showed fluorescence for the IgG isotype, 8 for the IgM isotype, and 3 for the IgA isotype.

Conclusions: Flow cytometry is a tool that provides accurate diagnosis of PAD-negative warm autoimmune hemolytic anemias, which are often challenging for conventional diagnosis.
