Translational integration of molecular and cytogenetic biomarkers in Hematology: contributions from the Institute of Hematology and Immunology

Authors

Abstract

Introduction: For 60 years, the Institute of Hematology and Immunology (IHI) of Cuba has progressed through different stages in the development of molecular diagnostics, aiming to provide accurate, timely, and accessible results to all Cuban patients with suspected hematological malignancies. In the period 2018-2025, the Institute has transitioned from conventional techniques to quantitative molecular biology platforms to implement a translational biomarker panel that allows for risk stratification, monitoring of residual disease, and therapeutic decision-making.

Objective: To describe the laboratory's experience in the implementation, validation, and future application of cytogenetic and molecular biology techniques for the diagnosis of acute leukemias, chronic myeloproliferative neoplasms, and other hematological malignancies, with an emphasis on the translational approach.

Methods: A narrative review was conducted in the PubMed, SciELO, and Google Scholar databases, along with an institutional document analysis, on the experience of the IHI's Molecular Biology and Cytogenetics Laboratory in implementing translational diagnostic techniques. The search was limited to the period between January 2018 and April 2025, with no language restrictions. Original articles, reviews, technical reports, and conference abstracts generated by the laboratory were included. The results were grouped by nosological entity and synthesized in narrative and tabular form. Active projects were described according to institutional documents.

Conclusions: The IHI laboratory demonstrates that it is possible to achieve molecular diagnostic standards in a resource-constrained environment by prioritizing cost-effective technologies and adopting an implementation science approach.
