Kidney involvement in multiple myeloma: two cases report

Abstract

Introduction: Multiple myeloma is a neoplasm of plasma cells that usually presents with kidney failure, anemia, bone pain, and hypercalcemia; however, it can also have other clinical manifestations, such as nephrotic syndrome or acute kidney injury.

Objective: To describe two cases of multiple myeloma and renal involvement, demonstrated by kidney biopsy, with different clinical presentations.

Case presentation: The first case was a 53-year-old male patient who presented with anemia and increased creatinine, required dialysis, did not present hypercalcemia, and underwent bone marrow aspiration with 60% plasma cell infiltration. The second case was a 70-year-old female patient who presented with edema and proteinuria, who underwent a kidney biopsy that reported light chain amyloidosis and was subsequently diagnosed with multiple myeloma.

Conclusions: Multiple myeloma can manifest as myeloma cast nephropathy or AL amyloidosis, so a kidney biopsy is important for diagnosis and treatment.
